A digital form solution built when government legislated information capture for healthcare providers - patients complete forms on their own devices via unique, one-time links, before they even arrive.

The challenge

During the pandemic, government legislated that certain information had to be captured by health care providers. The default was a clipboard and a shared pen in reception, which was both a hygiene risk and a delay on every single appointment.

What we built

• A digital solution that lets clients capture their information on their own devices, removing shared pens and clipboards.
• Unique, one-time-use links per client for added hygiene and safety.
• A flow designed so clients complete their form at their leisure, before they visit, rather than in the waiting room.

The outcome

Providers met the legislated capture requirement without turning reception into a bottleneck. Clients fill the form in before they leave home, on their own phone, so appointments start on time and there is nothing to hand across the desk.
